Generally, we don’t need extra fuel for exercise for activities under 1 hour, as our main meals should provide us with enough energy.

Still, if you need something, keep it simple, like half a banana, an apple, or a slice of toast. Sip water throughout.

If you feel nauseous or another side effect gets uncomfortable, there’s no shame in stopping. There’s always the next day.

For those experiencing joint discomfort, low-impact options work well: swimming, stationary cycling, chair-based exercises, yoga, and resistance band work.

How hard should exercise feel on Mounjaro?

Tirzepatide raises resting heart rate by an average of around 3 beats per minute in weight management trials1, with some variation by dose2.

That makes the standard ‘220 minus your age’ formula for calculating a target heart rate unreliable while you’re on the medication.

A simpler approach is to pace yourself by perceived effort, sometimes called RPE (rating of perceived exertion). The scale runs from 1 to 10:

  • If the gym feels overwhelming, then I’ll do a 10-minute session at home
  • If I feel nauseous during exercise, then I’ll switch to gentle stretching or yoga
  • If my usual routine feels impossible, then I’ll do 10 wall push-ups

Consistent, lower-intensity movement across the week does more for your health than occasional hard sessions, especially while your body adapts to the medication.

Preventing muscle loss

When you lose weight quickly, you’re more likely to lose some muscle along with fat4.

In the SURMOUNT-1 DXA substudy, around 26% of the weight lost on tirzepatide was lean mass, compared with 25% on placebo4.

For an 80 kg adult, that’s roughly 96 to 128 g spread across meals, or a palm-sized portion of chicken, fish, eggs, Greek yoghurt, tofu, beans, or lentils at each meal.

Keep eating protein on rest days too, as muscle repairs between sessions. Aim for 20 to 30 g within an hour or two of a strength session to support recovery.

Strength training

Compound movements that work several muscle groups at once give you the most return for the time. A simple home routine:

  1. Wall push-ups: 10 to 15 reps
  2. Sit-to-stands from a chair: 10 to 15 reps
  3. Bodyweight squats: 10 to 15 reps
  4. Resistance band rows: 10 to 15 reps
  5. Plank from your knees: hold for 15 to 30 seconds

Do 2 to 3 rounds, resting about a minute between exercises, twice a week.

If you’re just starting, set a minimum goal of one push-up or one squat, then build from there.

Warm up and cool down

  • Weeks 5 to 8: Finding your new normal. You may be able to push a bit harder now. Try different activities, such as swimming or cycling. Compare how you feel to a few weeks ago, not to before you started Mounjaro.
  • After 2 months: Energy usually improves once you’re on a steady dose. Exercise may still feel harder than before you started the medication, and that’s normal. Expect another adjustment period each time your dose increases, as side effects often return briefly at higher doses.
