Second Nature: published peer-reviewed research
Second Nature has published programme-specific outcomes in peer-reviewed journals. Our 2025 study in JMIR Formative Research examined 12-month outcomes for participants using semaglutide with our behavioural support programme and found1:
- 19.1% average weight loss at 12 months for participants who maintained an active subscription
- 77.7% achieved at least 10% weight loss
- 61.5% achieved at least 15% weight loss
- 69% reported no side effects by month 12
Separately, an NHS-published evaluation in the BMJ found that Second Nature achieved substantially higher weight loss than four other providers in the National Diabetes Prevention Programme.3
These are completer analyses rather than intent-to-treat analyses, which means they reflect outcomes for people who remained engaged with the programme.
Boots: established clinical team, limited published outcomes
Boots’ weight-loss service is led by a named clinical team that includes GPs, pharmacists, and a BANT-registered nutritionist. Their clinical lead for weight loss, Dr Sam Wellappili, is a GP with training in nutritional therapy and lifestyle medicine.4
Boots reports that 90% of their weight-loss patients are happy with the service.4 However, they haven’t published programme-specific weight-loss outcomes in a peer-reviewed journal, so it’s not currently possible to directly compare clinical results between the two providers.
Their coaching partner, Liva Healthcare, works with the NHS on diabetes prevention and has its own evidence base for digital health coaching, though this is separate from the Boots weight-loss service.

Second Nature
Second Nature pairs medication with a structured behaviour change programme developed by dietitians, psychologists, and behavioural scientists. The programme uses the COM-B model (capability, opportunity, motivation) and runs across four phases over 12 months or more.1
Day-to-day support is provided by registered dietitians and nutritionists (HCPC or AfN registered), available five days a week via in-app messaging. You’re also placed in a small peer support group with others at a similar stage, which provides accountability and shared experience.
The programme covers nutrition (using a balanced plate model rather than calorie counting), physical activity, sleep, and the psychological aspects of eating behaviour, with daily educational content delivered through the app alongside coaching.
Boots Online Doctor
Boots offers support through several channels. Their primary coaching is delivered through the Boots Online Doctor Coach app, built in partnership with Liva Healthcare, a digital therapeutics company that also works with the NHS on diabetes prevention.5
Through Liva, you’re assigned a health coach who provides personalised advice via video consultations and in-app messaging. Coaches include qualified dietitians, nutritionists, and health coaches trained in behavioural psychology.5
Boots also offers a free, self-paced 10-week programme in their Tracker app, covering psychology, nutrition, sleep, metabolism, and motivation. Aftercare and weight maintenance
Research shows that around two-thirds of people regain weight within a year of stopping semaglutide without ongoing support.6 This makes aftercare one of the most important factors when choosing a provider.
Both providers have developed approaches to address this, though they differ in structure.
Second Nature
Transition support is built into the programme itself. The later phases of Second Nature’s programme focus on consolidating habits and preparing for life after medication.
If you and your prescriber agree to reduce or stop medication, your dietitian continues to support you through that transition within your existing programme.
Boots
In March 2026, Boots launched a free 12-month aftercare programme, which they describe as the first of its kind from an online healthcare provider in the UK.
It was developed in response to NICE guidance recommending at least 12 months of structured follow-up after stopping weight-loss treatment.2
The aftercare programme includes unlimited messaging with Boots Online Doctor clinicians, monthly emails with expert resources on nutrition and activity, access to webinars and podcasts, weight tracking through their app, and free in-store weigh-ins. It’s available to patients who used the Boots weight-loss service for at least six months before stopping treatment.2
