Most people lose a modest amount in their first month on the Wegovy pill, often around 2-3% of their body weight.

Your first month is spent on the lowest 1.5 mg dose, which is there to help your body adjust to the medication rather than to produce the greatest weight loss.2

For someone weighing 100 kg, an average loss of 2 to 3% amounts to roughly 2 to 3 kg, though the range is wide, and some people lose less in the early weeks.

Most of the weight loss tends to come later, as the dose increases over the following months towards the 25 mg maintenance dose.

The Wegovy pill is licensed for adults with a BMI of 30 or higher, or between 27 and 30 with a weight-related condition such as type 2 diabetes or high blood pressure, and it’s available in the UK on a private prescription if you’re eligible.2

The Wegovy pill is a once-a-day tablet, so the first month is mainly about getting used to taking it correctly, adapting to any side effects, and adjusting to changes in appetite.

Most people notice a change in how hungry they feel before they experience significant weight loss.

In the first week or two, many people find their appetite reduces and the ‘food noise’, the constant background thoughts about food, begins to quieten.

Visible weight loss usually occurs in the second half of the month and is typically gradual.

Why your first month is on the 1.5 mg starting dose

The Wegovy pill is taken at four doses in turn: 1.5 mg, then 4 mg, then 9 mg, then 25 mg, with at least a month at each one.2

You spend your whole first month at 1.5 mg, which is the lowest of the four.

This starting dose isn’t likely to support significant weight loss in most people. It’s there to help our body get used to semaglutide, which reduces the risk of nausea and other digestive side effects.2

Most of the weight loss comes from the higher doses, particularly the 25 mg maintenance dose you reach after around four months.

A low amount of weight loss in the first month is expected and isn’t a sign that the medication isn’t working for you.

How to take the Wegovy pill correctly

How you take the Wegovy pill significantly affects how well it works because the body absorbs it properly only under specific conditions.2

Take it once a day, first thing in the morning on an empty stomach, with no more than a sip of plain water.2

Then wait at least 30 minutes before eating, drinking anything else, or taking other tablets.2

Swallow it whole, and don’t break, crush, or chew it.

If you eat too soon after taking it or take it with food, much less of the medication is absorbed, which reduces its effectiveness.

Building this into a fixed morning routine is one of the most useful things you can do in your first month, because taking the medication consistently will increase its effectiveness.

How much weight loss to expect after the first month

In the OASIS 4 trial, a Phase 3 study (large, late-stage human trials that decide whether a medication is approved), adults taking the 25 mg tablet lost an average of 13.6% of their body weight over 64 weeks, compared with 2.2% on a placebo.1

That 13.6% is the average across everyone who took part, including people who stopped early, so it reflects a realistic average you’d expect in real-world conditions.1

Among those who stayed on the pill and reached the maintenance dose, the average weight loss was 16.6%.1

Each dose increase tends to lead to greater weight loss, with the highest average weight loss observed at the 25 mg dose.

Side effects in the first month

The first month is when side effects are most likely, because our body is meeting semaglutide for the first time.

The most common digestive symptoms are nausea, constipation, diarrhoea, and a feeling of fullness or mild stomach discomfort.6

For most people, these are mild and settle within a few weeks as the body adjusts to the 1.5 mg dose.6

The slow, four-step dose increase is designed to keep these side effects manageable, which is part of why the first month stays at the lowest dose.2

We recommend a whole-food diet that provides enough protein, fibre, fat, and complex carbohydrates, and limits ultra-processed foods.

A simple guide is the balanced plate: half vegetables, a quarter protein, and a quarter complex carbohydrates such as wholegrain bread, brown rice, or potatoes, plus a serving of fat.

Aim for a portion of protein roughly the size of your palm at each meal, from foods like eggs, fish, chicken, beans, lentils, or Greek yoghurt.

As a daily guide, that’s around 1.2-1.6 g of protein per kg of body weight.

Does the Wegovy pill cause less nausea than the injection?

Not especially. Because both contain semaglutide, their side-effect profiles are broadly similar, and nausea is among the most common effects of either.6

The slow dose increase on the tablet is designed to keep nausea manageable, just as the injection increases each month.

What’s the worst day on the Wegovy pill?

With the weekly injection, some people experience a peak in side effects within a day or two after their jab.

The Wegovy pill is taken every day, so the level in our body stays steadier, and there’s no single ‘worst day’ in the same way.

Does eating less help me lose more weight in the first month?

You will naturally eat less on the pill as it reduces your appetite. So, it’s crucial to focus on the quality of the meals you’re eating to ensure you’re providing your body with the essential nutrients it needs.

It’s essential that you don’t eat too little, as this can increase the risk of muscle loss, fatigue, hair loss, and nutrient deficiencies.

It’s better to develop a lifestyle that supports sustainable weight loss in the long term than to try to lose weight as quickly as possible.

If you stop the Wegovy pill suddenly without having built the habits needed to maintain your new weight, some weight regain is likely.

In a trial of semaglutide, people who switched to a placebo regained around 7% of their body weight over the following year, while those who kept taking the medication lost a further 8%.4

This pattern of regain is consistent across the trials investigating what happens when people stop taking the medication suddenly.
