A Summary Care Record (SCR) is an electronic record of important patient information, created from GP medical records. They are used by authorised staff in other areas of the health and care system involved in the patient's direct care.
It is mandatory for our clinicians (independent prescribers) to review your SCR as part of their overall clinical review to be able to prescribe your medication. They will also reference your SCR in future to check your ongoing eligibility and suitability for the medication-supported programme. By joining the programme you are agreeing to your SCR being reviewed.
