Summary Care Record

A Summary Care Record (SCR) is an electronic record of important patient information, created from GP medical records. They are used by authorised staff in other areas of the health and care system involved in the patient's direct care.

It is mandatory for the clinicians (independent prescribers) to review your SCR as part of their overall clinical review to be able to prescribe your medication. They will also reference your SCR in future to check your ongoing eligibility and suitability for the medication-supported programme. By joining the programme you are agreeing to your SCR being reviewed.

Please note:

  • As a patient you can opt-out and revoke your consent to your SCR being reviewed. To do this please contact our prescribing team directly at 03330 34 24 38 or email careclinics@pharmalogic.co.uk.
  • If you choose to do this, the lead prescriber will then determine whether your medication and programme can safely be continued, or whether you need to be off-boarded and referred to another provider or your GP.
