Summary Care Record

A Summary Care Record (SCR) is an electronic record of important patient information, created from GP medical records. They are used by authorised staff in other areas of the health and care system involved in the patient's direct care.

The prescribing clinical team must review your SCR as part of its overall clinical review before it can prescribe your medication. For Foundayo, this is Second Nature's clinical team. For other medication programmes, this may be Blueco Healthcare's clinical team. The responsible team may also reference your SCR in future to check your ongoing eligibility and suitability. By joining the programme you are agreeing to your SCR being reviewed.

Please note:

— As a patient you can opt-out and revoke your consent to your SCR being reviewed. For Foundayo, email Second Nature at medication@secondnature.io. For another medication programme, contact Blueco Healthcare on 03330 34 24 38 or at service@blueco.care.
— If you choose to do this, the lead prescriber will then determine whether your medication and programme can safely be continued, or whether you need to be off-boarded and referred to another provider or your GP.
